When you are seeing several different health practitioners from a variety of healing backgrounds, there are times when you might feel that everyone is not “on the same page”, or that you and your health practitioners would benefit from a more coordinated and integrated approach.
GOAL
The goal of a Patient Care Conference (PCC) is that you and all your health practitioners have a common understanding and plan for helping you meet your health goals.
THE PCC
Before a PCC, you complete a comprehensive Patient Wellness Survey PWS. If you are new to IMC we recommend a preliminary visit with either one of our medical doctors or the naturopathic doctor (This is not part of the PCC formal process and is not covered by the PCC fee, but usually can be billed to your insurance). Your PWS is sent to ALL of the practitioners at IMC and any of your other practitioners that you choose. With this you get the advantage of a fresh perspective from a variety of health practitioners who work in a variety of health modalities.
After reviewing your PWS, practitioners often give suggestions about getting old records or suggesting new lab information that would be helpful to you. After this comment period, all practitioners who reviewed your PWS will be invited to the PCC.
Your primary contact at IMC will present an overview of your history and situation. Each attendee at the PCC will outline the possibilities of their modality or approach to your specific situation. Practitioners who are seeing you can share their insights into your healing path and progress. Recommendations and possibilities are discussed among all the practitioners.
REPORT
The outcome of the PCC is a written, staged, treatment plan based on the information that comes out of the PCC. This is reviewed with you either in person or by phone. The parts of the plan that you feel most comfortable with can then be implemented - at the pacing that works best for you.
TIMING
PCCs are held at IMC weekly.
We need about 3 weeks to get your PWS back, distribute it to providers and get their initial feedback, and then schedule the PCC to focus on your PWS.
We typically have 10-12 professionals involved including 20-40 min for each of their review and comment about your PWS and an hour of each of their time for the PCC.
COST of Patient Care Conference
The cost is $650. In some cases we can bill insurance for the review process following the PCC if it is done face to face at IMC. That portion is usually billed at $120.
